Zusammenfassung
Es wird über einen Fall von korrosiv verursachter, hypopharyngo-zervikaler Ösophagusstriktur
berichtet, wo sich im Laufe der Dehnungsbehandlung eine öspohago-bronchiale Fistel
und anschließend ein Lungenabszeß entwickelt hatte. Nach der Drainage des pulmonalen
Abszesses kommunizierte die ösophageale Fistel auch mit der Außenwelt. Die ösophageale
Fistel und der thorakale Prozeß heilten nach substernaler Plastik mit der rechten
Kolonhälfte (Pharyngo-ileo-colo-gastrostomie) und auch die Schluckfähigkeit der Kranken
wurde wiederhergestellt.
Summary
A case of hypopharyngo-cervical stricture of the oesophagus caused by corrosives is
reported in which an oesophago-bronchial fistula followed by a pulmonary abcess developed
during dilatation. The oesophageal fistula showed communication with the skin after
drainage of the pulmonary abcess. Both the oesophageal fistula and the thoracic process
healed after substernal plastic with the right half of the colon (pharyngo-ileo-colo-gastrostomy).
The patients ability to swallow was also restored.
Résumé
Il s'agit d'un patient présentant une sténose de l'oesophage sous le pharynx. L'origine
est due à une brûlure provoquée par un produit corrosif. Suite au traitement de dilatation
il apparaît une fistule oesophago bronchique et pour terminer un abcès pulmonaire.
Après drainage de l'abcès pulmonaire la fistule oesophagienne communique avec l'extérieur.
Pour guérir cette fistule oesophagienne et ce processus thoracique nous procédons
à une plastie sous sternale au moyen de la moitié du colon droit (pharyngo-iléo-colo-gastrostomie).
La déclutition du patient est également rétablie.
